Creating a new template
Evaluation templates are templates from which specific evaluation forms will be created for employees.
For an overview of the created templates, go to the HR > Employees evaluations tab in the navigation menu.
Then click the Evaluation management button and select Evaluation templates.

Creating a template
To create a new template, click the Add evaluation template button.
Then choose whether you want to:
- Use a preset scenario
- Start from a blank template

1. Use a preset scenario
If you select Use a preset scenario, a list of available assessment scenarios will be displayed.
Select the scenario that best suits your needs - an overview of the settings for the selected scenario will appear in the panel on the right, where you can edit the evaluation title.
If you selected a scenario for a three-phase assessment with objectives, determine who will set the objectives in the initial phase:
- Employee - goals are set by the employee and approved by the manager.
- Manager - goals are set by the manager in cooperation with the employee.
Then click the Create template button.

Then proceed in the same way as with the Start from a blank template option.
2. Start from a blank template
- Enter a Title (required).
- Select Evaluation instructions:
- Single-phase - in a single-phase assessment, no goals are set, i.e. after selection, the Contains goals and Who will enter goals fields are automatically hidden.
- Three-phase - if you choose the three-phase evaluation option, then choose whether the evaluation will include goals and who will set the goals. Goals can be set by both the evaluating manager and the evaluated employee.
- Fill out the Evaluation instructions (will appear in the header of the evaluation form).
- Alternatively, fill in the Note.
- Save by clicking the Add evaluation template button.

After saving the template, 4 tabs will appear:
- Basic settings
- Questions for employees
- Questions for the manager
- Schedule regular evaluations
Basic settings
Here you will find basic information about the evaluation.
Use the Options button to copy or delete the rating if necessary.
For detailed instructions, see the article Managing templates.

Questions for employee/manager
You can add evaluation questions to a saved template on the Questions for employee and Questions for manager tabs. The procedure is the same on both tabs.
Go to the desired tab and click the Add question button.

A form will open on the right, which you must fill out according to the following points:

- Question - Enter the question text, which you can format using the integrated text editor.
- Question subtitle - you can add a subtitle to the question if necessary.
- Type - select the type of response. This field specifies the data type of the response, i.e. whether the response will be text, number, choice of options, etc.

- To be answered in phase - here you can specify at which phase of the evaluation process the question will be displayed.

- Sections - you can sort your questions into any number of sections. In this case, first click the Add section button. A form will open in which you can enter the name of the section and its order, and then save it. After creating a section, you can then select which section the question should be placed in.

- Section order - determines the order in which questions will be displayed in a given section.
- Visible to employee - the switch is only displayed for questions for the manager.
- Required - if the switch is on (green), it means that the employee/manager must answer the question, otherwise the evaluation process cannot proceed to the next step. The mandatory question is then marked with an asterisk in the evaluation.
- Save the question by clicking the Save button.
All saved questions are displayed in a table.
The number in the circle on the tab indicates the total number of questions entered.

For each question in the table, you will find buttons for editing (pencil symbol) and deleting (cross symbol).
You can also edit the section name, change its order, or delete it here.
